Getting ready for your appointment
In order for us to provide the best care for you, it’s important that you bring the following items to your visit:
• Your insurance information and insurance card
• Photo ID
• Referral form, if applicable
• A list of your current medications
• Medical records from prior visits that you think may be helpful
• Any pertinent x-rays or lab reports
• Your patient registration forms which can be found below
For telemedicine appointments please log on to your patient portal account here and fill out the Medical History and Surgical and Allergies forms under the Questionnaires tab. You can also update your demographic information on the portal under the My Account tab. If you need help signing up for a patient portal account please contact our office.
